Kakarla Classification

The Kakarla classification was published by Kakarla et al. in 2008[^1] and is used to classify the position of external ventricular drains into three different grades.

Kakarla Grade

Position designation

EVD position

Grade I

Optimal position

Ipsilateral frontal horn and tip in the third ventricles

Grade II

Suboptimal position in non-eloquent brain tissue

Contralateral Lateral ventricle, Corpus callosum, or Interhemispheric fissure.

Grade III

Suboptimal position in eloquent brain tissue

Brainstem, Cerebellum, Internal capsule, Basal ganglia, Thalamus, occipital cortex, basal cisterns.
