Orbitozygomatic craniotomy can be considered an extension of the pterional craniotomy. In orbitozygomatic craniotomy, an additional osteotomy is performed in the superolateral orbital region as well as in the region of the zygoma. This additional bony extension expands the subfrontal trajectory and reduces the need for brain retraction to reach the floor of the anterior and middle cranial fossae.
