The DESH (Disproportionately Enlarged Subarachnoid Space Hydrocephalus) sign is an imaging finding indicative of normal pressure hydrocephalus. It is characterized by enlargement of the cerebrospinal fluid spaces in the region of the Sylvian fissure and the basal cisterns. In contrast, the cerebrospinal fluid spaces over the convexity become progressively narrower1.
