Hunt and Hess Classification

The Hunt and Hess classification is used to categorize the severity of subarachnoid hemorrhages based on clinical symptomatology and additionally provides an assessment of the patient’s prognosis. The Hunt and Hess classification was published in 1968[^1].

Criteria

Perioperative Mortality (%)

Grade 1

Asymptomatic or minimal headache and slight nuchal rigidity.

0-5 %

Grade 2

Moderate to severe headache, nuchal rigidity, no neurological deficits other than cranial nerve palsy.

1-10 %

Grade 3

Drowsiness, confusion, or mild focal deficits.

10-15 %

Grade 4

Stupor, moderate to severe hemiparesis, early decerebrate rigidity, and vegetative disturbances.

60-70 %

Grade 5

Deep coma, decerebrate rigidity, moribund appearance.

70-100 %
