TICI Classification

The TICI (Thrombolysis in Cerebral Infarction) classification is used to categorize cerebral perfusion after thrombolysis has been performed[^1].

TICI Grade

Cerebral Perfusion

TICI 0

No perfusion. No anterograde flow beyond the site of occlusion.

TICI 1

Penetration with minimal perfusion. The contrast agent passes beyond the site of occlusion but cannot perfuse the entire vascular bed distal to the obstruction.

TICI 2

Partial perfusion. The contrast agent can pass beyond the site of obstruction and opacifies the vascular bed distal to the obstruction. However, the flow rate of the contrast agent as well as its outflow are significantly slowed compared with the healthy side.

TICI 2a

Only a partial filling (2/3) of the entire vascularized territory is visualized.

TICI 2b

Complete filling of all vascularized territories is visualized, but the filling is delayed.

TICI 3

Complete perfusion. Anterograde flow of the contrast agent is seen at the same speed as on the healthy side.
