Anderson and Montesano Classification

A commonly used classification for fractures of the occipital condyle is the Anderson and Montesano classification, which was published in 1988 [^1].

Fracture type

Description

Type I

Comminution of the occipital condyle due to axial force. Minimal or no displacement of the fracture fragments into the foramen magnum.

Type II

Fracture of the occipital condyle as an extension of a fracture at the foramen magnum. Most likely caused by direct force applied to the skull.

Type III

Avulsion of the occipital condyle due to the alar ligament. Most likely as a result of rotation and/or lateral flexion of the head. Unstable fracture.
